Structure Family Care
The Structured Family Caregiving program (SFC) with Health Force of Georgia offers financial and support services to Georgia families that deliver care at home for a senior or disabled person. You may meet SFC requirements and be eligible for financial compensation if:

You live with the person as the primary caregiver

Are related biologically as a daughter, grandson, etc. or are related through marriage as a daughter-in-law, son-in-law, etc.

The person you take care of is Georgia Medicaid-eligible and participates under the CCSP or SOURCE waiver program
Personal Care Assistant or CNA
Medicaid / Medicare
The in-home assessment leads to each client’s Personal Care Program. The Personal Care Program is a customized resource guide followed by our compassionate caregivers to deliver all day-to-day services for your loved one. The RN supervisor is responsible for making sure all service needs are met.
Daily-living services include:

Help with moving around the home

Private Pay
Private pay home health aide services are in-home care options that clients pay for out of pocket, providing personalized help with daily activities and medical needs without depending on insurance.
